A 69-year-old woman with a medical history of overweight BMI, coronary artery disease (CAD), hypertension (HTN), hyperlipidemia, and aortic stenosis presents with intermittent chest discomfort that has persisted for five days. The episodes, lasting under five minutes each, are diffuse, triggered by mild exertion, and relieved with rest within 3–5 minutes. She also reports exertional fatigue and shortness of breath. She denies any history of smoking, alcohol consumption, or drug use, as well as recent upper respiratory infections, viral illnesses, fever, weight loss, nausea, or vomiting.
