Case Problem Statement
A 45-year-old male with a history of heavy alcohol use, presents with marked anxiety, tremor, diaphoresis, and mild auditory hallucinations (clock ticking). He has been drinking alcohol excessively over the last 6 months (after a 2-year period of sobriety) and stopped abruptly 8 hours prior to ED arrival. His relapse was precipitated by work-related stressors, nonadherence to his alcohol treatment and AA, and associated marital discord. He smokes cannabis occasionally, but has been abstinent from all other drugs. Physical examination reveals a thin, diaphoretic, overtly agitated, anxious, and tremulous black male. P 110, BP 138/90, afebrile. He is mildly confused and disoriented to time (otherwise oriented). He exhibits impaired rapid- alternating movements, reduced light-touch and vibratory sensation of the feet and a broad-based gait.
